The relationship between coronary collateral circulation and visceral fat

Collateral circulation is really important as it is a potential alternative source meeting the need and becoming part of the activity when there is insufficiency for providing blood stream that will meet the needs of the heart in coronary vessels.These collaterals occurring as a response to advanced coronary stenosis and varying among the individuals having coronary artery disease stem from triggering factors.In this study, we wanted to share the relationship between visceral fat ratio, being one of effective factors of collateral growth, and coronary collateral circulation.The selection was made among the patients whose coronary angiography and diagnosed with critical stenosis in at least one of their coronary arteries with %90 and over in our clinic.In conformity with the previous studies totally 148 patients included were divided into two groups as between 1 and 9, and between 10 and over by specifying their visceral fat ratio with biyoelectrical impedence analysis(BİA) method.Collateral circulation was evaluated in accordance with Rentrop collateral classification.The ones, whose Rentrop grades were 0 and 1, were grouped as bad collaterals and the ones, whose Rentrop grades were 2 and 3, were grouped as good collaterals. In the analysis made in accordance with collateral classification, body mass index(BMI) and waist circumference averages were found as high in bad collateral group and this difference was found as meaningful in visceral fat ratio (p=0.011) and body mass index (p=0.040). In ROC analysis, we confirmed that increase in visceral fat level decreased collateral growth with %72.7 sensitivity and %58.5 specificity.We showed that visceral fat ratio was in a stronger position over collateral growth when it was compared with BMI and wais circumference.This study reveals that the increase in visceral fat ratio is an independent factor related to bad collateral growth.It could be provided that the patients may live healthier and longer with by performing practical, quick, cheap and noninvasive examination of visceral fat ratio with BIA method in daily clinical use.Key Words: Collateral Circulation, Visceral Fat Ratio, Coronary Artery Disease
