Özet (EN)
Corrosive ingestion is a disease of the industrial age. Caustic esophageal injuries lead to stricture formation. Although a number of agents have been tried experimentally to prevent strictures, few have gained clinical application. The purpose of this study is to evaluate the effects of stent placement before stricture development following corrosive esophageal burns and using balon dilatation and cutting balon after stricture development on stricture treatment and weight lost.Forty rats were allocated into five groups. Caustic esophageal burns were created as described by Gehanno. Group K was instilled only with saline. Group Y was injured and untreated. Group B was injured and they underwent balon dilatation on the third and fourth week. Group KB was injured and on the third week cutting balloon treatement was applied. Group S injured and subsequently silicon stent was placed in the same session. On the 6th week, stenosis index (SI), collagen deposition, and hydroxyproline content (HP) were determined in distal esophageal segments. Statistical analyses were done. Although weight loss was determined only in Group B except group Y( 238,87 ± 15,95 GR vs 233,83 ± 19,01 GR), this lost was not as obvious as in group Y. When the groups were examined according to stenosis index (SI) ;between group Y and B, there is an significant difference in favor of Group B. Although there was no difference between Group B and Y in respect to weigth before the procedure ( P=0,318 P>0,05), there was statistically significant difference after(p=0,002 p<0,05). When SI of Group KB was compared with group Y,it was statistically lower in group KB. Weigth gain in group S was similar with group K, however it was statistically higher when compared with Group Y and also SI was lower according to group Y.In conclusion, in strictures due to corrosive esophageal burns evaluation either for SI or weigth lost, stenting after esophageal burn and using cutting balloon after stricture development results in favorable effect. Therefore, there is practicality in clinic practice, however this must be supported by other experiments and human studies.
