The relationship of life quality with sleep quality and depression in chronic renal failure patients

Abstract (EN)
Objectives: CKD causes high mortality and morbidity and significantly reduces the quality of life of the person. It is imperative for quality of life management to elucidate the psychological factors associated with kidney disease. The aim of this study is to evaluate the relationship of quality of life with sleep and depression in patients with CDK. Materials and Methods: Patients with a diagnosis of degree 3-5 CDK who applied to Manisa Celal Bayar University Nephrology Outpatient Clinic and volunteers who were admitted to the General Internal Medicine Outpatient Clinic with age and gender matched and no chronic diseases were included in our study. A Questionnaire Form was applied to paticipant and the Questionnaire Form was designed in four sections. In 1st part "Information on Socio-Demographic Features", in the 2nd part SF-36 (short form-quality of life scale) scale for determining the quality, in the 3rd part Beck Depression Scale was used to determine the severity of depression, in the 4th part Jenkins Sleep Scale was used to sleep quality. Results: In our study, sleep disorders and depression were found to be higher in our patients than in the general population. It was observed that patients with low quality of life had more sleep quality disorder and depression and this was significant (p<0.05). It was observed that the quality of life score was significantly higher in control group than the patients, and depression and sleep disturbance were less (p<0.001). In the control group, depression and sleep disturbance were negatively correlated with quality of life scores. In the stage-3 CKD group, sleep disturbance was not significantly associated with quality of life and depression, while depression was negatively associated with general health (p=0.028) and social function scores (p=0.023). In the stage 4 CKD group, sleep disturbance was negatively associated with general health (0.021) and depression was negatively associated with physical function (p=0.021), pain (0.043) and role difficulty (0.002). In the stage-5 CKD group, sleep disturbance was negatively correlated with physical function (p=0.001), general health (p=0.002), vitality (p=0.012), emotional role difficulty (p=0.019) score, and depression was negatively correlated with general health (p=0.018), emotional role difficulty (p=0.001) and mental health scores (p=0.002). A significant correlation was found between sleep disturbance and depression in the stage-5 CKD group and showed a positive correlation (p=0.001). Conclusion: It is seen that psychosocial support is inevitable for chronic kidney diseases, especially for patients undergoing dialysis. Behavioral therapy and psychological support recommendations should be offered to patients in a broader framework rather than merely treating the disease.
