Evaluation of after treatment changes in histolopathological findings of patients with chronic hepatitis B

Abstract (EN)
Chronic hepatitis B is a viral infection leading cause liver -related mortality . In this study, we performed liver biopsy in chronic hepatitis B patients receiving oral antiviral treatment with lamivudin, entecavir or tenofovir for at least three years and have evaluated the efficacy of oral antiviral drugs. Between March 2015 – March 2016, 79 patients were receiving treatment of lamuvidin (100mg/day) entecavir (0,5mg/day) or tenofovir disproksil (245mg/day) for at least three years were included the study with HBeAg positive or negative. 23 of the patients were using lamivudine, 21 of them were using entecavir and 35 of them were using tenofovir. All of the patients' control liver biopsy was performed. The patients' biochemical, serologic, virologic and histopathologic datas were recorded and compared with the data obtained following the use of oral antiviral therapy for three years or longer. All of the patients taking one of the oral antiviral for 3, 4 or 5 years ALT normalization in order were 60%, 73.3% and 72.2%; and virological response (HBV DNA <20 IU / ml) were 60%, 60% and 83.3% observed. HBsAg positive patients between pre-treatment and control measures were improved in patients receiving oral antiviral drugs in all three, but there was unremarkable change. In the lamivudine, entecavir, tenofovir group were recession in the histological activity index score (in sequence p=0.011 p=0.002 p=0.001). In the group of Lamivudine and tenofovir showed a significant improvement in fibrosis scores (p=0.033, p=0.001) unlike in the group receiving entecavir showed no significant improvement in fibrosis scores (p=0.090). Before the oral antiviral treatment both who were taking interferon and weren't showed significant improvement in terms of controlling HAI and fibrosis scores. In conclusion, in patients with long-term oral antiviral treatment field of biochemical, virological, serological and histological (histological activity index and fibrosis) improvement has been seen. Histopathological improvement is required for long-term use of oral antiviral drugs with constantly suppression on HBV DNA.
