A comparison of the stages of fibrosis with arfi elastography findings and histopathological results in patients with chronic hepatitis

Abstract (EN)
Aim: The aim of our study was to investigate the contribution of ARFI elastography to the diagnosis of fibrosis in chronic hepatitis cases compared with histopathologic findings. Material and methods: Between June 2018 and August 2018, 122 patients with chronic hepatitis who were referred to our clinic for liver biopsy were included. 10 shear wave velocity measurements were made in the right lobe of the patients' liver. The mean ARFI values were compared with fibrosis stages based on ISHAK classification. Results: The first comparison was between patients with stage ≤ F1 fibrosis and stage ≥ F2 patients. Sensitivity was 77 % and specifity was % 68 when the optimal cut-off value of two groups of ARFI elastography was 1.26 m / s. The second comparison was between two groups of patients with ≤ F2 and ≥ F3 fibrosis stages to determine significant fibrosis. The sensitivity and specificity were 85 % and 87 %, respectively, when the optimal cut-off value of two groups of ARFI elastography for e significant fibrosis was 1.45m / sec. The third comparison was between patients with stage ≤ F4 and fibrosis stage ≥ F5 patients to differentiate cirrhosis with other fibrosis groups. Sensitivity was 100 % and specificity was 84 % when the optimal cut-off value for ARFI elastography was 1.64 m / s in two groups. Conclusion: In our study, with ARFI elastography reliable results was obtained in identifying the presence of significant fibrosis and determining cirrhotic processes. ARFI elastography is a method that can take place biopsy in most cases of chronic liver disease. Key Words: Ultrasonography, ARFI, Chronic hepatitis.
