Preoperative and postoperative diffusion and perfusion magnetic resonance imaging findings of the patients with chronic subdural hematoma in evaluation of the brain parenchyma next to hematoma

Özet (EN)

Objective: In this study we aim to find changes on adjacent parenchymal brain tissue microcirculation improvement curve on clinical and radiological of early and late stage, after the hematoma removed. Method: In this study we included 25 patients who operated with chronic subdural hematoma admitted to our facility during 2013-2014. Patients with MRI diffusion, perfusion flow and volume shooting was done with General Electric brand 8 tesla MRI device. Findings: The hematoma removed with two different surgical procedure as burr hole drainage and craniotomy. The patients diffusion and perfusion MRI values, shift and hematoma thickness, ages, left and right side difference, surgical technic differences parameters were examined. In the both surgical technics, diffusion and perfusion MRI values, shift and hematoma thickness in the late postoperative period showed improvement compared to preoperative and early postoperative period. Early postoperative values showed improvement compared to preoperative values. In patients who underwent burr hole drainage method, improvement was observed to be faster and more effective. Only early postoperative values in perfusion MRI measurements showed decrease compared to preoperative values. Results: All patients showed clinical and radiographic benefits from the removal of hematoma. Using MRI is more valuable than CT, especially if you don't want surprises of differential diagnosis and clinical follow up to subdural hematomas. However, the diagnose and follow up with MRI needs more times
