T lymphocytes sub groups in chronic urticaria

Primarily in the diagnosis of chronic urticaria, physical, allergic, pseudo-allergic and infectious causes are excluded. The unknown facts are called chronic idiopathic urticaria (CIU). More than 50% of patients with chronic urticaria is in this group.Recently,in some patients with CIU, against high-affinity IgE receptor in mast cells and basophils the ? subunit (anti-FcRI) or versus IgE, IgG autoantibodies were detected.This fact is called chronic autoimmune urticaria. In the patients with CAU autoantibodies are functionally active and lead to the release of histamine in vitro,in healthy donors?basophils and mast cells in the skin. These antibodies are detected by using autologous serum skin test (ASST). The most development about chronic urticaria is that approximately 30-50% of patients are associated with autoimmunity and this type of disease is called autoimmun urticaria (AIU) not as idiopathic. CD4+ T hepler subsets are important in pathogenesis of autoimmun diseases. CD4+T cells can be divided into sub groups which have different diffrentiation and functional features. In present Th1, Th2, T reg, Th17 are known as four major T lymphocete subsebts.In this study we aimed to provide a progress in the pathogenesis of CIU by using ASST and peripheral blood mononuclear cell culture methods and by determining in CIU, autoimmunity and its relationship with helper T lymphocyte subsets and cytokines. In our study we have done the research on 20 ASST(-) and 20 ASST(+) of 40 patients with CIU. Peripheral blood mononuclear cell culture was performed to determine cytokine patterns. In cell culture supernatants, IL-4, IL-17, IL-23, IL-10, TGF-ß, IFN-? were studied by ELISA method.In the study, IL-4, IL-10, IFN-?, IL-17, IL-23 levels of patients were lower than control group?s levels statistically significantly. Although not statistically significant TGF-ß avarage level of patients was lower than control group?s levels.With these results and literature data, it can be said that Th1, Th2, Th17 ve Treg cell function are not enough or they are compulsive in patients with CIU. From the perspective of ASST(-), especially in patients with CIU, IL-4, IL-10, IL-17, IL-23 levels were determined signifacantly low therefore in this type of patients groups,we think that Th2, Th17 ve Treg functions are not enough or they are compulsive.In this study, in PKMHK supernatants cytokine levels was evaluate by using ELISA method witout specific or non specific stimulus for lymphocytes. In future studies,in PKMHK cytokine levels can be evaluated more clearly by stimulating specifically or non specifically for T lymphocytes.
