Testis sparing surgery in small testicular masses

Testicular cancer which is the most common tumor in young male patients, has an excellent prognosis with proper treatment. This situation increases the expectations of patients in terms of quality of life. Because of small benign pathologies, radical inguinal orchiectomy may be over-treatment for particular patients. Testis sparing surgery for favorable patients is promising in terms of both oncological and functional outcomes. The aim of this study was to evaluate functional and oncological outcomes testis sparing surgery. The data of inguinal orchiectomy performed between 1/1999 and 10/2018 were reviewed and 32 cases who underwent testis sparing surgery were selected. The contralateral testis was normal in 24 patients, 5 had bilateral synchronous tumors, and 3 had solitary testicle. 9 patients had germ cell tumor and 23 had non-germ cell tumor. There was no significant change in testosterone levels before and after surgery. No patient required testosterone replacement, and no patient complained of erectile dysfunction. Testis sparing surgery with the proper patient selection can be used in small testicular masses to preserve fertility and to protect patient from testosterone replacement. Keeping in mind that testis sparing surgery is not a standart treatment for patients who has a normal contralateral testis, except the solitary testis tumor or metachronous/ synchronous tumor, patients should be informed in this respect.
