INTRODUCTION: The 25% of traumatic cases that apply to emergency have rib fractures and lung contusion.The lung contusion that emerges in 30%-75% of the major thorax is the serious injury which has a high mortality and morbidity rate.The mortality rate given as 11% in an isolated intense contusion, rises 22% when additional injuries come out (6).In our study after the lung contusion we aimed to evaluate the protective effectiveness of inhale steroid budesonide and one of the antiinflammatory cytokines IL-10 .MATERIALS and METHODS: 55 of Sprague Dawley male rats were used in this study.Via performing isolated blunt thorax trauma, lung contusion was created for the other groups except the Sham group. The intensity of the trauma was determined as 1.45 joule. To the groups of budesonide and IL-10, budesonide as orogastric and IL-10 as intraperitoneal were given 30 minutes before the trauma and on the first, second day after the trauma. Only blunt thorax trauma was performed to the controlled group. In the begining, on 1st, 2nd, 3rd days groups blood gases, blood glutatyon, malondialdehit and TNF alfa values; lung tissue histopathological evaluation and bronchoalveolar lavage cells counted results were recorded.DIAGNOSES: Following the trauma in five rat (9%) mortality was observed. For budesonide, IL-10 and sham groups PaO2 and SatO2 values were found higher compared to the control group (p<0,05). A great difference in PaO2 value among budesonide, IL-10 and sham groups weren?t observed (p>0,05). No difference in SatO2 value between budesonide and IL-10 groups was seen (p>0,05) and the SatO2 rate in these groups were lower compared to the sham group (p<0,05). PaCO2 value in sham group was higher than other groups (p<0,05) and it wasn?t different among controlled, IL-10 and budesonide groups (p>0,05).In budesonide, IL-10 and sham groups malondialdehit and TNF alfa values were lower compared to the controlled group (p<0,05). For budesonide group TNF alfa value was observed higher than IL-10 group (p<0,05). And no difference in TNF alfa value between IL-10 group and sham group was seeen. Also malondialdehit value for budesonide, IL-10 and sham groups wasn?t different (p>0,05).The glutatyon value among budesonide, IL-10 and sham groups was higher than the controlled group (p<0,05). In budesonide group the glutatyon value was lower compared to the IL-10 group ( p<0,05). And that showed no difference between IL-10 and sham groups (p>0,05).The average of lung damage in budesonide and IL-10 groups was found higher than sham group but it?s lower than the controlled group (p<0,05). Bronchoalveolar lavage inflammatory cells number were diagnosed higher in the controlled group than other groups (p<0,05).RESULTS: It?s observed that in the lung contusion of the subject rats, the use of budesonide and IL-10 reduced both intensity of lung contusion and the inflammatory reaction to minimum degree.
