Affecting factors on perioperative nausea and vomiting in patients under laparoscopic sleeve gastrectomy operation

Abstract (EN)
Post-surgical nausea and vomiting (PONV) is a bothersome complication that can occur after both open and laparoscopic surgeries due to anesthesia and usually within 24 hours of the operation. In this study, it was aimed to compare the group given Laparoscopic Sleeve Gastrectomy and given 0.9% sodium chloride (Group S) and the group given 0.9% sodium chloride + 5% dextrose (Group D) in terms of PONV. Information about age, gender, height, weight, BMI, ASA score, accompanying comorbid conditions and the targeted fluid management method applied to the patients included in our prospective, randomized controlled clinical study were recorded. Based on the targeted fluid management technique, the patients were randomly divided into two groups as 0.9% sodium chloride + 5% dextrose (n=100) and 0.9% sodium chloride (n=101). Preoperative and postoperative laboratory values were examined and fasting blood sugars were recorded. In addition, postoperative nausea-vomiting scores, antiemetic and analgesic consumption and first mobilization time of the patients were recorded at the postoperative 0 minutes, 30 minutes, 6 hours and 24 hours. A total of 201 patients, 100 (49.8%) Group D and 101 (50.2%) Group S, were included in the study. There was no significant difference between the groups in terms of demographic data (sex, age, weight, height, BMI), presence of additional disease, ASA, duration of surgery, pre-induction blood glucose, intraoperative opioid consumption, postoperative analgesia need and total fluid amount (p>0,05). The extubation blood glucose level of Group D was found to be significantly higher than that of Group S (p<0.001). The incidence of PONV in Group D (48%) was significantly higher than that in Group S (25.7%) (p=0.001). In conclusion, the incidence of postoperative nausea-vomiting in Group D was found to be significantly higher than Group S in our study. Avoiding dextrose-containing fluids in operations performed on such morbidly obese patients will help reduce the rate of post-operative nausea-vomiting.
