İdentify the relationship between the quality of care and postoperative pain care in patients with major orthopedic surgery

Objective: The purpose of this study identify the relationship between the quality of care and postoperative pain care in patients with major orthopedic surgery. Method: The research is cross-sectional and descriptive type. The research was conducted between January 2019-2020.with 99 patients who underwent total hip and knee replacement in the Orthopedics and Traumatology departments. The data were collected with "Demographic Data Form" prepared by researcher in accordance with the literature and "Strategic and Clinical Quality Indicators in Postoperative Pain Management Questionnaire", "Revised American Pain Society Patient Outcome Questionnaire." Results: After total knee replacement and total hip replacement surgery, the mean first pain score (0-6 h) patients feel when they come to the clinic was 6.82 ± 2.91, and the mean pain score during the interview was 3.08 ± 2.09. The first pain score patients feel when they come to the clinic and severe pain level were higher in female patients. Sleep and activity, affective interference and advers effects caused by postoperative pain were higher in female patients. On the second postoperative day, the quality of pain care and perception of care was higher. İt was found that, statistically significant negative difference was between postoperative perception of care and worst pain score (r=-0.291), percent time spent in severe pain (r=-0.387), pain related sleep (r=-0.238), activity interference (r=-0.206) (p<0.05). İt was found that, statistically significant negative difference was between percent time spent in severe pain and participate in decisions about pain treatment (r=-0.339), satisfied pain treatment (r=-0.257) (p<0.05). Conclusion: Basic data on the quality of care in pain control and postoperative pain experiences in orthopedic patients were obtained in our study. Nursing practices and quality of care should be require improved to increase satisfaction with the postoperative pain and decrease the negative effects caused by pain.
