Evaluation of early results of isolated ASD surgery with median sternotomy and minnie thoracotomy

Abstract (EN)
PURPOSE: In this thesis study, it was aimed to evaluate postoperative and early results of closing of atrial septal defects with median sternotomy and mini thoracotomy methods. METARIAL and METHODS: In this study, the data of 30 patients who underwent ASD and treated surgically between January 2013 and December 2016 at Department of Cardiovascular Surgery of Atatürk University Medical Faculty were evaluated. Thirty patients in our study (17 female, 13 male) ranged 18 to 44 years to age. Postoperative and early stage results of patients undergoing median sternotomy and mini thoracotomy as surgical methods were evaluated. RESULT: The most common symptoms in patients are dyspnea, fatigue, palpitation, chest and back pain. 63,3% of the patients had fossa ovalis, 16,7% had supravalvular, 10% inferior caval and 10% persisting type ovale type defects. There are 17 patients in NYHA I before surgery and 13 patients in NYHA II. In the postoperative period, all of the patients were NYHA I. Patients who underwent mini thoracotomy discharged on average of 1.47 days from intensive care unit and on average 6.7 days from the hospital. Patients who underwent median sternotomy discharged on average of 1.67 days of intensive care and on average at 7.3 days. In both groups, right atrium and right ventricle diameters decreased significantly in preoperative and postoperative ECO evaluation. However, there is no statistically significant difference between the groups themselves. No mortality due to surgical treatment was observed. CONCLUSION: We believe that mini-thoracotomy, which has become widespread in recent years, is the first choice in the treatment of ASD, not only because of the positive cosmetic results but also because of the contribution to patient healing.
