Memedeki mikrokalsifikasyonların karakterizasyonunda dijital zumlama ile magnifikasyon grafisinin etkinliklerinin karşılaştırılması

Purpose: The purpose of this study is to compare the diagnostic effectiveness of the digital zooming technique with magnification graphy in microcalcifications. Materials and Methods: Between January 2013 and March 2015, 57 patients who applied to our unit and were diagnosed with microcalcifications on diagnostic or screening mammography and had a definite diagnosis were included in the study. The cases were evaluated in terms of calcification morphology, distribution, BI-RADS category and probability of malignancy as percentage on digital mammography and magnification images by three radiologists with different mammographic imaging experience. Kappa and ROC analysis were used in the statistical evaluations. Results: There was a low-to-moderate level of compliance for morphology and distribution parameters between the radiologists both on the magnification graphies and digital zooming. Sensitivity and specificity were %61-84 and %32-68 for digital zooming and %92-100 and %34-55 for magnification graphy, respectively.The sensitivity of the magnification graphy for three radiologists was about 20% higher than the digital zooming. According to BI-RADS evaluation, diagnostic efficacy of magnification graphies was higher than digital zooming for all three radiologists but that was statistically not significant in radiologists 2 and 3 (p <0,5666). In the evaluation performed according to percentage probability of malignancy; again the diagnostic efficacy of magnification graphy was higher than dijital zooming for all readers. However, that was statistically significant just for one radiologist (p <0,542). Discussion: This retrospective study shows that; it is also possible to evaluate microcalcifications by digital zooming. However, the diagnostic efficacy of digital zooming, especially cancer detection sensitivity, is lower than the magnification graphy. Therefore, especially for benign appearing focal calcifications detailed evaluation with magnification graphy must be performed before considering routine or close follow-up.
