Comparison of the efficacy of IV zoledronic acid treatment in patients with metastatic breast cancer patients with isolated bone metastases and patients with bone and visceral organ metastases

Abstract (EN)

OBJECTİVE: Iv zoledronic acis is sequestered into bone tissues and has a positive effect on survival by preventing bone morbidities. In our study, we aimed to evaluate the survival of iv zoledronic acid between the group with isolated bone metastases and the group with bone and visceral organ metastases. METHODS: In our study, 111 patients over the age of 18, who were followed up in the Medical Oncology Division of Dokuz Eylul University Medical Faculty Hospital after 1.1.2010, and who had a report of iv zoledronic acid and received iv zometa treatment for at least 1 year were included in our study. At the same time, patients with a second primary malignancy and insufficient pathology samples were excluded from the study. The patients were reviewed retrospectively. RESULTS: Of 111 patients, 61 were in the group of patients with isolated bone metastases and 55 with bone and visceral organ metastases, and the mean age of the patients at the time of diagnosis was 51.1. In addition, 53.2% of the patients were found to be in the premenopausal patient group at the time of diagnosis. While there was no significant difference between the median progression-free survival of both groups, when the median overall survival was evaluated, the median survival time of the isolated bone metastasis patient group was 121.6 months, while the median survival time of the bone and visceral organ metastasis patient group was 66.3 months. When the mortality rates of both groups were evaluated, the mortality rate in the patient group with isolated bone metastases was 59%, while the mortality rate in the patient group with bone and visceral organ metastasis was 82%. CONCLUSİON: When the overall survival times of the patients with isolated bone metastases and the patients with bone and visceral organ metastases were compared, a significant difference was found in terms of isolated bone metastasis. In addition, when the mortality rates of both groups were evaluated, a significant difference was found, which was higher in the patient group with bone and visceral organ metastases. In addition to bone metastasis, patients with visceral organ metastases develop metabolic problems and organ failure due to visceral metastases in the later period, and as a result, the patients' CTs are interrupted or they cannot take the full dose, as well as the burden of organ dysfunction as a result of visceral organ metastasis and the performance score of the patients. There may be a significant difference in both mortality and overall survival time between the two groups, depending on the decrease in survival rate. As a result; Comprehensive and supportive studies are needed to evaluate the overall survival between patients with isolated bone metastases and patients with bone and visceral organ metastases, in which they are divided into groups according to characteristics such as ER, PR, HER2, grade, Ki-67 and recurrence score, which affect prognosis and surveillance.
