Effects of N-acetylcysteine in contrast nephropathy in rats

Abstract (EN)

29 9) İngilizce Özet, Summary N-acetylcysteine(NAC)has yielded some promising results recently in the prevention of RCIN. In this study, we aimed to analyze the structural and functional effects of NAC in RCIN. 28 wistar rats were randomized into 4 groups. 1) Control, 2) Contrast, 3) Contrast+NAC, 4) NAC. All rats were deprived of water for 24 hours then contrast medium (ioxaglate, 10 mL/kg) were administered to 2nd and 3rd groups; NAC (50 mg/kg) was introduced enterally to 3rd and 4th groups. After 4 days rats were sacrificed. Creatinine clearance was calculated. In tissue samples, Malondialdehyde(MDA) was quantified. H&E and PAS stained slides were examined by light microscopy. Each tubular crossection from all images were scored; Grade 1: Preserved brush border without necrosis; Grade 2: loss of brush border without necrosis, grade 3: Loss of brush border and necrosis. Their frequencies were compared. Basal creatinine and creatinine clearance were similar in all groups. Serum creatinine presented statistically significant increase only in contrast group (0,6±0,1 vs 0,7±0,2; p<0,05) although it was clinically nonsignificant. Tissue MDA levels were similar in all groups. When control and contrast groups were considered; Grade 2 (13,8%±1,5 vs 42%±1,4; p<0,05) and Garde 3 (0% vs 40%±2,1; p<0,05) lesions were significantly more frequent in contrast group. With the administration of NAC in conjunction with contrast; the frequencies of Grade 3 (40%±2,1 vs 20,2%±0,86; p<0,05) lesions were found to be decreased by half and that of Grade 1 (16%±1,5 vs 37%±1,5; p<0, 05) lesions were found to be increased. As a conclusion NAC attenuates development of tubular necrosis in kidney induced by contrast medium administration.
