The investigation of colorimetric capnometry events in the correct placement of nasogastric tube

Abstract (EN)
The research is a methodological study conducted with the aim of examination of the colorimetric capnometry method used in determination of correct placement of the tube in nasogastric tube application (NGT). The research was conducted in Neurology Intensive Care Unit in the Faculty Hospital of Medicine in Ege University between the dates of August 2014 and January 2015. The population of the research is the NGT applications conducted to the patients hospitalized in the intensive care unit where the research is performed and the sample is 40 tubes application forming the population and suitable for the sample criteria. In the data collection, case report form colorimetric capnometry device, radiological film, stethoscope and stopwatch were used. For the determination of placement locations of NGT, three methods as colorimetric capnometry, auscultation and radiological method (gold standard) were used. In each tube application, the placement location of tube was evaluated with three methods and the results obtained were tested. In the evaluation of the data, Fisher's Exact Test, Cohen Kappa correlation test was used. In conclusion of the research, it is found that the age mean of the patients to whom NGT was applied in the research was 72.37±14.82 year, 55% of them was women; the clinical diagnosis of 82.5% is cerebrovascular disease, the awareness status of 27.5% is apathy, 72.5% of them H2 receptor antagonist, 40% of them is connected to intubated and ventilator, NGT is applied to 27.5%, NGT is applied to 57,5% successfully at the first entrance, and any respiratory distress sings are not seen during tube application in 50% of them. In the research, according to the evaluation results of placement location of the tubes in NGT applications, it was detected that the placement of 90.0% them in stomach in the auscultation method of NGTs applied, 95% of them in colorimetric capnometry method, 92.5% of them in radiological evaluation method. In the analysis performed, in verification of the placement of feeding tube, the adaptation rate of colorimetric capnometry method with radiological method is 97.5% and this adaptation was found to be statistically significant (p<0.05). The usage status of consciousness status, H2 receptor against are not affected by NGT placement evaluation results of colorimetric capnometry method of the variables like intubating (p>0.05). In the determination of the true placement of the feeding tube of colorimetric capnometry, it was found that the sensitivity was found to be 1.00, specificity of it as 0.667, and the positive predictive value as 3, negative predictive value as 0, sensitivity of auscultation method as 0.89, specificity as 0.0, positive predictive value as 0.89, negative and negative predictive value as ∞. In the determination of feeding tube placement, the total time mean used in the measurement only with colorimetric capnometry is 16.30±4.59 seconds; the mean for the evaluation for radiological placement was detected to be 68.35±34.80 minutes. As a conclusion, the research data indicates that colorimetric capnometry measurement method is a method consistent with radiology in determining the tube placement in feeding tube applications and it takes less time than the radiology method.
