Evaluation of factors affecting treatment response and disease-FRRE survival in gastric cancer patients receiving neoadjuvant therapy

Objectives: It was aimed to examine the parameters that predict pathological response, overall survival and disease-free survival in patients with locally advanced gastric cancer who received neoadjuvant therapy. Materials and Methods: Patients with gastric cancer, who were treated and followed up at Dokuz Eylül University Medical Oncology Department between 2010 and 2021, were retrospectively searched. Eighty-five patients over the age of 18 who were accepted as locally advanced stage and received neoadjuvant therapy without secondary malignancy were included in the study. Results: The effects of patients' demographic data, pathological stages, tumor markers, systemic inflammation markers on pathological response, overall survival and disease-free survival were analyzed. While there was no statistically significant finding in predicting the pathological response from the parameters checked before treatment, the pathological response was found to be statistically significant in the group with a low PLR value after treatment (p<0,05). In the analysis made over the mean values, it was found statistically significant that the prognosis was worse in the group with high SII after treatment (24,6 vs 40,7 months, p= 0,015), and the overall survival was shorter in the group with high PLR values before and after treatment. GPS and post-treatment PLR values were statistically significant in terms of disease-free survival. It was observed that the disease-free survival time was shorter in the group with higher PLR values after treatment (10,1 vs 22,2 months, p=0,04). Conclusion: When the findings were assessed together, it was seen that systemic inflammation parameters had a predictive value for the prognosis in gastric cancer patients receiving neoadjuvant therapy. No parameter was found that predicts pathological response before neoadjuvant therapy, which may affect patient selection before treatment.
