Investigation of cardiac enzyme values and their effectiveness in diagnosis on patients applied with seizures

Abstract (EN)

Epilepsy is one of the most common neurological disorders. In pediatric patients, these seizures may present as syncope, febrile convulsions and status epilepticus. In addition to routine biochemical markers, electroencephalogram and seizure monitoring in these patients, we investigated cardiac enzymes in children with seizures to emphasize the importance of cardiac enzymes in the mechanism of seizure formation and their importance in follow-up and treatment. In our study, patients admitted to Fırat University Pediatric Emergency Department between August 2019 and August 2021 and followed up in the Department of Neurology with a diagnosis of seizure were included. Patients diagnosed with seizure were divided into 3 groups of 25 patients each according to seizure type and duration; status epilepticus, syncope and convulsion. The ck, ckmb, troponin, hemogram, biochemistry, age, gender, previous seizure history, seizure duration, and cranial imaging findings were investigated retrospectively. Patients with ck, ckmb and troponin values within 24 hours of seizure onset were included in the study. It was observed that 33.3% of the CONVULSION group, 50% of the SYNC group and 92% of the STATUS group did not have a first seizure. Seizure duration at presentation was 0-10 min in 58.3% of the CONVULSION group, 41.7% of the SYNC group, and 64% of the STATUS group had >15 seizures. It is seen that 50% of the CONVULSION group and 79.2% of the SYNC group had no specific imaging findings, while 68 of the STATUS group had specific imaging findings. Troponin level was 0 in 45.8% of the CONVULSION group, 95.8% of the SYNC group and 56% of the STATUS group. The higher troponin value of status and convulsion patients compared to syncope patients may be directly proportional to the duration of seizures. The troponin value higher than 0 in 44% of status patients and 54.2% of convulsion patients showed that seizures may be correlated with troponin value. This suggests that cardiogenic factors may be effective in the mechanism of seizure formation in SE and convulsion patients.
