Abstract (EN)

ABSTRACT Aim: Proteinuria is an important laboratory test showing kidney dysfunction and renal progression. Although there has been a lot of research on proteinuria measurement methods and comparisons in different groups of patients, there has been no study showing the correlation between protein excretion in 24 hours urine samples and protein / creatinine ratio in instant urine samples in obese and morbidly obese individuals. We aimed to compare the 24 hours urine samples and protein / creatinine ratio in instant urine samples in obese and morbidly obese individuals. Mateial and Methods: We have included obese patients with a BMI ≥ 35 older than 18 years who applied to Inönü University Medical Faculty Nephrology, Endocrinology and General Surgery Obesity (Bariatric Surgery) polyclinic. Before the study, written approval was obtained from the Ethics Committee of Inonu University Medical Faculty. Written consent was obtained from all the patients to be employed. Patients' anamnesis was taken and physical examinations were performed. Demographic data such as age, sex, as well as BMI, height and weight were recorded. In addition, complete urine examination, microprotein and creatinine in spot urine, and proteinuria amount in 24 hour urine were measured. Patients' who was concluded to study, 24 hours urine sample proteinuria and instant proteinuria were compared. Findings: : A total of 178 patients were enrolled, 134 females and 40,1 ± 11,5 average years of age. The distribution of proteinuria amounts of the patients who was concluded to study is; in 136 subjects amounts of proteinuria was < 150 mg / day, in 33 subjects was 150-500 mg / day (minimally icreased), in 7 subjects was 500-3500 mg / day (non nephrotic proteinuria) and in one subject was > 3500 mg / day (nephrotic proteinuria). A moderate-weak correlation was observed in the analysis made (p= 0,003 r= 0,223). In addition 24 hours urine proteinuria and spot urine protein / creatinine ratio were compared in patients (n=22) who had proteinuria ≥ 0,2 g / day. According to this, no significiant correlation was observed between the amount of proteinuria calculated in the spot urine and 24 hours urine proteinuria (p=0,064). Results: We found a very weak or statistically insignificant association between the instant urinary protein / creatinine ratio and protein excretion in 24 hours urine sample. Therefore, looking at 24 hour urine protein seems to be a good method to detect daily protein excretion in obese and morbid individuals. Key words: Proteinuria, obesity, protein / creatinine ratio
