Özet (EN)
Aim: In our study, it was aimed to evaluate the clinical, radiological and functional results of pediatric supracondylar humerus fractures operated in our clinic and to compare them with the literature. Materials and Methods: The radiological, clinical and functional results of 78 patients who were operated on with the diagnosis of supracondylar humerus fracture in the Department of Orthopedics and Traumatology of Karadeniz Technical University between 2012-2021 were analyzed. Baumann angle, lateral humerocapitellar angle and bearing angle were evaluated during radiological examination. During the clinical examination, the elbow joint range of motion and elbow bearing angle were measured according to the McRae method and these were evaluated according to the modified flynn cosmetic and functional criteria. K (kirschner) wire was used as the surgical fixation. Results: In our study, 25 (32.1%) of 78 patients were female and 53 (67.9%) were male. fracture was present. The mean age of operation was 6+3 years. The postoperative follow-up period was between 1 and 12 years, with an average of 4+3 years. According to Gartland classification, 17 patients (21.8%) were type 2, 45 patients (57.7%) type 3 and 16 patients (20.5%) type 4. The number of detected columns was divided into two groups as single and double. There were 22 patients (28.2%) who had single colon fixation and 56 patients (71.8%) who had double colon fixation. Twenty-seven (34.6%) of the patients were fixed with closed reduction and 51 (65.4%) open reduction with k-wire. The average splint removal time of the patients was determined as 5+1 weeks, and the average pin removal time was 5+1 weeks, and then exercise was started. 11 of our patients had nerve symptoms before surgery. At the final follow-up examinations, it was observed that all of them showed full recovery. All patients were evaluated according to the modified flynn cosmetic-functional criteria. Cosmetic; 68 patients (87.2%) were found to be excellent, 9 patients (11.5%) good, and 1 patient (1.3%) bad. Functional; 57 patients (73.1%) were excellent, 12 (15.4%) good, 4 (5.1%) moderate, 5 (6.4%) bad. Modified flynn cosmetics of patients who underwent open reduction; 42 patients (53.8%) were found to be excellent, 8 patients (10.2%) good, and 1 patient (1.2%) bad. Modified flynn cosmetic for closed reduction patients; 26 patients (33.3%) were found excellent, 1 patient (51.2) good. Modified flynn functional of patients who underwent open reduction; 33 patients (42.3%) were excellent, 10 patients (12.8%) good, 4 patients (5.1%) fair, 4 patients (5.1%) bad. Modified flynn functional of patients who underwent closed reduction; 24 patients (30.7%) were found to be excellent, 2 patients (2.5%) good, and 1 patient (1.2%) bad. Modified flynn cosmetics of patients with single colon fixation; 19 patients (24.3%) were found to be excellent, 3 patients (3.8%) were found to be good. Modified flynn cosmetics of patients with double colon fixation; 49 patients (62.8%) were excellent, 6 patients (7.6%) good, and 1 patient (1.2%) bad. Modified flynn functional of patients with single colon fixation; 19 patients (24.3%) were excellent, 1 patient (1.2%) good, 2 patients (2.5%) moderate. Modified flynn functional of patients with double colon fixation; 38 (48.7%) patients were excellent, 11 (14.1%) good, 2 (2.5%) moderate, 5 (6.4%) poor. Conclusion: Clinical, radiological and functional results of supracondylar humerus fractures fixed with open or closed reduction. Whether the number of detected colons is single or double, clinical, radiological and functional results are close to each other. Depending on his experience and experience, the surgeon can treat his patients with the best surgical method. Key Words: Supracondylar Humerus Fractures, Modified Flynn Criteria, Open And Closed Reduction.
