Effect of clinicopathological findings on loco-regional recurrence and survival in oral cavity cancers

INTRODUCTION: The aim of this study was to investigate the effect of clinical features (age, sex, etc.) and pathological findings (tumor depth, volume, differentiation, perineural invasion, etc.) on survival and recurrence development in patients with oral cavity cancers, And to determine related parameters. MATERIALS AND METHODS: Clinical and pathological findings of patients diagnosed with oral cavity cancer at Celal Bayar University ENT Department between January 2006 and April 2015 were retrospectively analyzed. 129 patients were included in the study. Existing data; Sex, age and smoking status were grouped as patient characteristics. Features of the disease; Characteristics belonging to the tumor, features belonging to the local spread of the tumor and features belonging to the regional spread were grouped under the main headings. The features belonging of the tumor are the position, size and volume of the tumor and differentiation information. Features belonging to the local spread of the tumor are mandibular invasion, perineural involvement and lymphovascular involvement tumor; And lymph node involvement and extracapsular involvement were grouped as features of regional spread. Surgical marginal status, stage distribution and local, regional recurrence and distant metastasis and status information were recorded and the effects on disease-free survival and overall survival were examined. RESULTS: Lymph node involvement was found to be an effective major risk factor for local recurrence, regional recurrence and status of the patient (p<0.05). Mandibular invasion was the most effective risk factor for distant metastasis and lymph node involvement was the second most effective risk factor (p<0.05). If the tumor depth is> 4 mm, it is the most important predictive factor for lymph node involvement (p<0,05). Furthermore, the presence of perineural invasion was also predictive of lymph node involvement (p<0.05). Five-year overall survival and perinural invasion were the major risk factors for 5-year disease-free survival(p<0,05). Tumor volume ≥ 0.64 cm3 is an effective risk factor for 5 years overall survival and perineural invasion is an effective major risk factor for 5 years disease-free survival(p<0,05).
