Abstract (EN)

Evaluation of liver fibrosis in moderate and severe psoriasis vulgaris patients AIM: Psoriasis vulgaris is a disease that is not limited to the skin and may be associated with metabolic syndrome and non-alcoholic fatty liver disease(NAFLD). Recent findings suggest that cytokine-mediated inflammation may be the common link. Early detection of NAFLD is essential for personalized treatment approaches. Our study aim is to evaluate liver fibrosis noninvasively by retrospectively investigating moderate-severe psoriasis vulgaris patients followed in our outpatient clinic. MATERIALS AND METHODS: 200 moderate-severe psoriasis patients and 50 volunteers were included in the study. The patients' examination, imaging results and data in the patient files were recorded. PASI was used to evaluate the severity of psoriasis. The patients' NAFLD-FS, FIB-4, De Ritis rates at the time of maximum PASI were calculated. According to these scores, liver fibrosis risks and metabolic syndrome prevalence were determined. All data acquired were compared with the control group. To compare, Patients and volunteers were also divided into those under and over 40 years of age. RESULTS: There was no difference between psoriasis patients and the control group in terms of age, gender, smoking and alcohol use rate. The prevalence of metabolic syndrome in the psoriasis group was 31,5 higher than the control group. Moreover, low average HDL value, Insulin and triglyceride values and waist circumference were high. 27% of the patients were in the risk group for liver fibrosis according to the NAFLD fibrosis score, 52% according to the De Ritis ratio, and 17% according to the FIB-4 score. NAFLD fibrosis and FIB-4 scores were significantly higher in psoriasis patients compared to the control group. Maximum PASI value, disease duration and presence of psoriatic arthritis in psoriasis patients; It did not show a relationship between metabolic syndrome and the risk of liver fibrosis. With a history of INH use; No correlation was determined between the presence of metabolic syndrome and liver fibrosis scores. Fatty liver on USG was not higher in psoriasis patients than in the control group. CONCLUSION: The risk of NAFLD, metabolic syndrome, liver fibrosis and other comorbidities increase in psoriasis patients. Regular follow-up of psoriasis patients at risk for metabolic syndrome and liver fibrosis with liver ultrasonography and non-invasive biomarkers; It can help in early diagnosis and selection of appropriate treatment.
