The prognostic value of ERCC1, fascin1, P53 in ovarian surface epithelial tumors

Abstract (EN)

Ovarian cancer is the 5th frequently seen cancer type in the World and is the 7th in line among the cancers causing death. Ovarian surface epithelial tumours morphologically have substantial heterogenic structure. So diagnosis is based on morphologic and immunohistochemical data. In this study, expression of ERCC1, fascin1 and p53 by using immunohistochemical methods, morphological and clinical prognostic parameters and the relation of these features were examined in 92 patients who were diagnosed as ovarian surface epithelial carcinoma between 1999 – 2007. The patients were treated with chemotherapy consisting of platin and paclitaxcel after radical surgery. In this study mean age was 52.5. Of the 92 cases 42 cases (45.7 %) were serous type, 18 (19.6%) endometrioid type, 8 (8.7%) musinous type, 6 (6.5%) clear cell type, 4 (4.3%) undifferentiated and 14 (15.2%) mixed type. Ovarian carcinomas are tumours that have substantially malignant prognosis. In this study 31 cases lost their lives. And also in our study, significant relation was found between survival and Silverberg grading system, lymph node metastasis, omentum metastasis, positive peritoneal cytology similar with the literature. Silverberg degree was determined as independent prognostic factor. No significant relation was found between the immunohistochemical expression of ERCC1 and prognostic parameters and survival. Over tumours diagnosed most frequently at late stages, the application of adjuvant therapy in our opinion effects the survival period. Apart from this in the literature in many studies evaluations were made according to molecular studies but in our study for ERCC1 expression only immunohistochemical technique was used. In wider series of studies based on more clinical information, molecular techniques, we think we shall obtain more meaningful results from our studies. As an immunohistochemical the relationship between fascin1 expression and age found to be meaningful. In accordance with the literature among the histological types in serous tumours, expression was found to be high and in high degree serous carcinomas it was found to be even higher. We think that the fascin expression and survival period not being found meaningful may be due to tumour heterogeneity. Immunohistochemically the relationship between P53 expression, Silverberg degree and rupture of the tumour capsule is meaningful. Apart from this in serous tumours expression was found to be higher comparing with other types. We think that the reason for P53 being found not related to survival is ovarian carcinomas being frequently applied of adjuvant therapy effecting the survival period. In our study many morphological parameters, strong relationship with prognosis came to our attention. Silverberg degrees strength info seeing the staying alive and being the single independent predictive factor shows that it has a stronger place than FIGO degree. The strength of morphologic findings in foreseeing the staying alive is one of our results that support literature. We think that the reason of finding meaningless results from the analysis of leading molecules in prognosis is related with the effect of adjuvant chemotherapy in survival. Our necessity for studies that includes molecular examination techniques with complete clinical data is continuing. Key words: ovarian carcinoma, prognosis, ERCC1, fascin1, p53
