Abstract (EN)

The increasing incidence of Pancreatic Neuroendocrine Tumors (PNETs) is one of the most complex and interesting topics in modern oncological endocrinology in recent years. PNETs bring with it the need to be evaluated multidisciplinary With their wide clinical presentation range, different biological behaviors and variable prognoses. Although advances have been made in the diagnosis and treatment of PNETs due to developments in molecular biology, the management of PNETs maintains its complexity and the search for this issue continues. In our study, we wanted to make studies that we hope will guide us in the biological behavior of PNETs and, accordingly, in prognostic predictions. We determined the 6-month, 1, 2 and 5-year survival in patients diagnosed with PNET between January June 2006 and September 2019 at Dokuz Eylül University Hospital; age at diagnosis, gender, clinical presentation, tumor localization, tumor size, tumor stage, obtaining surgical resection or non-surgical sample, presence of liver and/or distant metastases, tumor obtained by Ki-67 index and mitotic count We aimed to evaluate the relationship between the degree of hemoglobin at the time of diagnosis, white blood cell count, serum albumin, serum amylase level, and CRP level. During the specified period, 68 patients over the age of 18 who were diagnosed with PNET through the hospital information system were identified. Four patients were not included in the study due to the lack of pathology samples in our hospital, and nine patients because they were diagnosed with a second primary malignancy. The data of the remaining 55 patients were scanned, and the end date of the follow-up was determined as the date of death for the patients who died during this period, and as 31.05.2021, when the data collection process for the other patients ended. Due to the updates in the staging and grading systems of PNETs in 2017, the tumor stage at the time of diagnosis was re-determined according to the WHO 2017 criteria, according to the mENETS Criteria, based on the pathology and radiology reports of the patients. The relationship between the independent variables' survival at 2 and 5 years and the calculated follow-up periods (months) were evaluated. In addition, the relationship between independent variables was also examined. SPSS 23 program was used to perform the analyses.Fisher's Exact test was used to analyze the relationships between categorical variables. The assumption of normal distribution was checked with the Shapiro Wilk test. In the analysis of the difference between the measurement values of the two independent groups, the Mann-Whitney U test was used when it did not comply with the normal distribution, and the Student's t-test was used when it did, and the relative risk scores were calculated using the Cox regression analysis method. When we compare it with 2 and 5 year survival; A statistical relationship was found between age at diagnosis, tumor stage, presence of liver and/or distant metastases, and CRP level from biochemical data. The median survival time of the study group was 149 (95% CI: 85.8-212.2) months. The 6-month, 1, 2, and 5-year survival rates were found to be 92.7, 89.1, 85.5, and 80, respectively.
