Compairment of gain of parallel multiple Z-plasty and opposite directed multipl Z-plasty with V-Y advancement flap-Experimental study

Z-plasty is the most used technicque for releasing of the contracture bants in plastic surgery.Main indications for z-plasty are: releasement of contractures, as incision type especially preventing formation of contractures in hand surgery and scar correction.Especially there are 3 situations that offend the eye. These are:1.Gaining length in contracted scar treatment.2.Facial scar treatment with changing direction of the common limb.3.Preventing formation of scars especially in ellective or urgent hand surgery procedures.The most convenient angle is 60? in planning z-plasty. Because with decrease of the angle circulation problems of the flaps can occur and the desired gain of length may not be obtained. İncreasing the angle can cause difficulties in transpositioning of the flaps.Single Z-plasty may not produce desired results for long contracture lines. This is due to shortening of transvers side. There may be no sufficient skin on both sides of common limb of the Z. Multiple Z-plasty is preferred for this cases. Besides using multiple Z-plasty in facial scar treatment causes better results.In this experimental study triple z-plasty which are parallel to each other are compared with triple z-plasty which are opposite to each other for elongation. 15 rats were used in this study. Weights of 250 gr were attached with adehesive tape to both inguinal regions of each rat. Afterwards parallel triple multiple Z-plasty totally 3 cm in lenght was applied to the right inguinal region, opposite triple muliple Z-plasty totally 3 cm in length was applied to the left inguinal region. Measurements were done immediately after the surgery and results were evaluated statistically. The length of skin gain obtained by opposite triple multiple z-plasty was more.In this study we demonstrated that skin gain obtained with opposite triple multiple Z-plasty is more than the skin gain obtained with parallel triple Z-plasty.
