Retrospective analysis of the effect of nivolumab treatment on systemic inflammation index, hemogram parameters, and uric acid/alt ratio in metastatic cancer patients receiving a PD-1 inhibitor (nivolumab)

Objective: Nivolumab has shown promising efficacy in various metastatic cancers, including renal cell carcinoma (RCC), malignant melanoma (MM), and non-small cell lung cancer (NSCLC). The Systemic Immune-Inflammation Index (SII) is used as a prognostic indicator and mortality-related marker in solid tumors and coronary artery diseases. In our study, we aimed to retrospectively investigate the effects of PD-1 inhibitors on systemic immune inflammation, hemogram parameters, and the uric acid/ALT ratio in metastatic cancer patients admitted to the Bolu Abant İzzet Baysal University Oncology Clinic. Materials and Methods: The laboratory and clinical data of 53 patients who applied to the Medical Oncology Clinic of Bolu Abant İzzet Baysal Training and Research Hospital between 2021 and 2024 were retrospectively analyzed. The demographic information, nivolumab usage status, systemic immune inflammation, hemogram parameters, uric acid/ALT values, and clinical data of the patients were examined. Results: Of the 53 patients receiving Nivolumab, 43 were male and 10 were female. The mean age of the patients was 68.62 ± 9.24 years. Twenty-nine patients were smokers, while 24 were non-smokers. Among the patients, 33 had NSCLC, 13 had RCC, and 7 had MM. All demographic data of the patients were compared with the Systemic Inflammation Index, Hemogram Parameters, and Uric Acid/ALT Ratio to assess treatment response. Logistic regression analysis revealed that the uric acid/ALT ratio was a significant predictor of treatment response. Conclusion: Systemic Inflammation Index, Hemogram Parameters, and Uric Acid/ALT Ratio were evaluated according to treatment response in metastatic cancer patients receiving Nivolumab. Logistic regression analysis revealed that the uric acid/ALT ratio was a significant predictor of treatment response. However, we believe that more comprehensive, multicenter studies are needed to obtain more conclusive results.
