Postoperative analgesic effects of bupivacaine and ketamine infused to incision line in pediatric cases undergoing inguinoscrotal surgery

Abstract (EN)

Postoperative pain which is caused by surgical trauma-induced tissue damage and accompaning to central and peripheral sensitization is a type nociceptive pain. The hormonal and metabolic responses to surgical stres are more severe in children.In our study, we aimed to evaluate analgesic effects of bupivacaine and ketamine infiltrated to incision line in children undergoing inguinoscrotal surgery by comparing among themselves and with the control group in recovery room and early postoperative period.This study was performed in 90 children undergoing inguinoscrotal surgery for undescended testes and hernia repair, with ASA I-II risk groups and aged between 1-7 years-old. The patients were randomly divided into three equal groups consisted of 30 cases.Before skin suturing at end of the operation, 0.25% bupivacaine ml cm-1 (2.5 mg ml-1) in cases of the Group B, 0.5% ketamine ml cm-1 (5 mg ml-1) in cases of the Group K and %0.09 NaCl ml cm-1 in cases of the Gruop S were infiltrated to per centimeter of the incisions.Systolik Arterial Pressure (SAP) values of the cases at 5 and 10 minutes in the recovery room were lower in the Group K than the Group S, it was low in intra-group comparison, according to preoperative values; after extubation, SAP value of the Group S was increased at 5, 10 and 15 minutes in the recovery room; and the values of the group K were found low at 10, 15 minutesin in the recovery room and 2 and 6 hours in the service.CHEOPS scores after extubation were found lower in the Group K than the Group S; according to the Group S, in the Group B it was found low at 10th, 15th minutes in the recovery room and 2nd hour in the service.As a result, we concluded that similar to local anesthetics, ketamine infiltraion into wound localisation provided adequate postoperative analgesia and also its use in this manner was safe and side effects were decreased.
