Clinicopathological significance of hyalin deposites detected in peripheral LYMPH nodes in pelvic and abdominal malign tumors

OBJECTIVE: Although hyaline deposits detected in radical dissections of abdominopelvic lymph nodes due to tumor have been the subject of some previous studies, the relationship of these organ malignancies with lymphatic spread has not been investigated. With this study, we aimed to determine whether there is a relationship between the presence of hyaline deposits in lymph nodes and lymph nodes metastasis, as well as plasma cell infiltration in tumor tissue and lymph nodes draining the tumor, and immunoglobulins produced in plasma cells and hyaline deposits and metastasis. METHODS: Forty-five cases of abdominopelvic organ carcinomas and radical dissection materials involving at least one of the pelvic, obturator, and paraaortic lymph nodes were included in the study, and the presence of hyaline deposits in the lymph nodes was evaluated. Plasma cell infiltration density in H&E sections and immunohistochemical staining, IgG, IgA, IgM, Kappa, Lambda release intensity were evaluated. RESULTS: In lymph nodes draining endometrial carcinomas, hyaline deposits were more prominent than in other abdominopelvic organ carcinomas(p<0,001). Significantly fewer metastases were observed in lymph nodes with hyaline deposits than in lymph nodes without deposits(p<0,001). As the intra and peritumoral plasma cell density increased, the frequency of lymph node metastases increased. There was no correlation between the density and ratio of immunoglobulins released from plasma cells and metastasis, and no accumulation of any of these antibodies in hyaline deposits. CONCLUSION: Whether the correlation between the presence of hyaline deposits in the lymph nodes draining the tumor, plasma cell density infiltrating the tumor and lymphatic metastasis in abdominopelvic organ tumors is also valid for other organ tumors and to reveal the metabolites that lead to the formation of hyaline deposits requires further studies on this subject.
