Abstract (EN)

Peripheral arterial diseas (PAD) is defined as total or partial obstruction of one or more peripheral artery as a consequence of atherosclerosis. PAD is an important cause of morbidity that also affects quality of life of the patient. Prevalance of PAD is closely related with age that it is about 5% before the age of 50, 10% around the age of 65 and 25% above the age of 80.Revascularisation is not recommended in 39% of the cases with severe ischemia. In such cases, amputation surgery is needed and extremities are saved in only 25% of the patients. Therefore, pharmacological treatment in PAD is very important.Iloprost treatment has been an important alternative method of treatment in the recent years. It has been reported that iloprost is effective in trophic healing of lesions, reduction of pain at rest, decrease of the need for amputation and decreased rate of mortality.Thirty patients (19 men and 11 women, mean age 60,7 ± 13,7 ) who have any chance of surgery or endovascular intervention with detailed examination were included to study. Iloprost infusion were applied via forearm veins 0.5 ng/kg/min. rate for 16 hours.In the present study, we have aimed to investigate effects of iloprost on indicators of endothelial function such as acetyl dimethyl arginin (ADMA) , serotonin and nitric oxide (NO) in patients with PAD. We have determined that there is a significant difference in serum ADMA (p= 0,001) levels of pre- and post-iloprost treatment. However, no significant changes were observed in serotonin (p= 0,82) and NO (p= 0,16) levels.We conclude that ADMA may be an important parameter in monitoring the prognosis and effects of iloprost treatment in PAD patients who had no chance of surgical treatment.Key Worlds: Iloprost, Nitric Okside, ADMA, Serotonin
