Comparison of the assessment of penile deformity in peyronie's patients with autophotography, intracavernosal injection and 3D CT

Abstract (EN)

Introduction and Aim: It is important to evaluate the penile curvature in an objective, precise and reliable method in the Peyronie's disease (PD) group. for this reason, it is important to evaluate penile deformity correctly in PD . In this study, the type of the deformity, direction, grade and parameters required for the planning of the penis operation were compared with autophotography, erection obtained with vasoactive agent injection, and evaluation with 3D CT in the erection. Materials and methods: Thirty-six patients with Peyronie's Disease were included in the study, who applied to the Istanbul Medical Faculty Andrology Department's polyclinic. Patients were divided into 5 groups according to their curvature directions and right, left, dorsal, ventral curvature, deformity and notch according to the presence of complex deformity. Patients were compared in terms of anatomic features such as direction skewness, angle, presence of notch, and clinical parameters associated with PD by three methods. Results: Age, duration of Peyronie's disease, body mass index, ASA score, DM, HT, ischemic heart disease, HL, smoking, palpable plaque presence, IIEF-5 EF score were not statistically different between the groups. When the general correlation analysis of the methods used to evaluate the curvatures of the patients in the three groups is made; The relationship between autophotography and CIS was found to be strong (rs: 0,72), and the relationship between autophotography and CT was found to be moderate (rs: 0.59). It was determined that the autophptpgraphy did not correlate with the ventral and left-sided group, while the intracavernosal injection was correlated with CT, all curvature groups. When CIS was accepted as the gold standard, it was concluded that the autophotography could be used safely in the group with dorsal and left curvature, and CT could be used very safely in the group with ventral and right curvature. Erectile function capacity was determined as a factor affecting the correlation of angles. In the group without erectile dysfunction, the correlation of CIS with autophotography increased moderately to strongly correlated (rs 0,72 to 0,93) when compared with the general group. It was determined that none of the clinical parameters related to general and PD affected the correlation of the method used Conclusions: As a result of the study, it was concluded that the best evaluation in PD was the evaluation with intracavernosal injection. Intracavernosal injection, CT and autophotography were followed. The autophotographic method was concluded to be useful in evaluating the patient group with good erectile function (n = 6, rs: 0,94).
