Evaluation of the relationship between pruritus in plaque type psoriasis patients and disease severity, systemic immunoinflammatory index and systemic inflammatory response index

Abstract (EN)

INTRODUCTION AND AIM: Psoriasis is frequently brought to the agenda with its itching aspect and significantly affects the quality of life of patients. It is also an inflammatory disease and neutrophils play a significant role in the onset and exacerbation of the disease. Our study aimed to investigate pruritus in patients with plaque psoriasis and to evaluate the relationship between pruritus and various features of the disease, such as disease severity, and to reveal its relationship with inflammation using parameters such as systemic immune-inflammatory index and systemic inflammatory response index. METHOD: The study is a retrospective cross-sectional study and 200 patients with plaque psoriasis were included. The examination results of the patients and patient information were recorded. The psoriasis area and severity index (PASI) score was used to assess the severity of psoriasis, the dermatology life quality index (DLQI) score was used to assess quality of life and the visual analog scale (VAS) score was used to assess pruritus. The neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), systemic ınflammatory response ındex (SIRI) and systemic ımmunoinflammatory ındex (SIII) rates obtained from c-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and hemogram parameters were used to assess inflammation. RESULTS: There is correlation between disease severity and VAS and DLQI scores. Results of CRP, ESR, SIRI increased with disease severity but did not show a significant relationship. There is correlation between disease severity and NLR, PLR, SIII ratios. Results of CRP, ESR, NLR, TLR, SIII, SIRI did not show an important change with pruritus. Also VAS and DLQI scores were related. CONCLUSION: Pruritus accompanies most patients with psoriasis, and itching affects the quality of life. In addition, there are many factors that affect itching and its severity. In psoriasis, which has a neutrophil-mediated inflammation cascade in its pathogenesis, the use of NLR, PLR, SIII and SIRI rates in practice is therefore being investigated. However, the fact that there was no significant relationship between pruritus scores and these rates in our study indicates that pruritus may not only be related to inflammation and that patient-specific conditions may also be effective. This may also help develop more personalized, effective follow-up and treatment strategies for patients. Keywords: Disease severity, inflammation, pruritus, psoriasis, quality of life.
