Evaluation of injectable trombosi̇t-rich fibrin efficiency in postmenoposal atrophic vaginitis

The aim of this study is to investigate the efficacy of injectable trombosit-rich fibrin (i-PRF) in the symptomatic treatment of atrophic vaginitis developing in the postmenopausal period, which covers approximately one third of a woman's life. Trombosits play a central role in wound healing and trigger the release of injectable trombosit-rich fibrin, growth factors and cytokines, triggering cell migration, tissue regeneration and wound healing. To this study; Thirty-five postmenopausal patients with atrophic vaginitis complaints such as vaginal burning, dryness, itching and sexual dysfunction were included. After approximately 18 ml of venous blood was collected from the patients, i-PRF was prepared by centrifuging at 700 rmp for 3 minutes. Injections were made into the first three centimeters of the posterior wall of the vagina at three points at one centimeter intervals. The application was repeated for the same patient twice with an interval of one month. Atrophic vaginitis symptoms of the patients were evaluated using female sexual function index (FSFI) and sexual quality of life scale (SQOL) questionnaires before the procedure, at the first and sixth months after the procedure. The mean age of the patients participating in the study was 54.1±5.5. Before and after the procedure, the sexual life quality scale and female sexual function index; When desire, arousal, lubrication, orgasm, satisfaction, pain and total scores were evaluated, it was determined that there was a highly significant improvement (<0.001). Trombosit-rich fibrin application; high success rate, ease of application technique, autologous prepared material, the absence of complications and side effects related to the procedure, short processing time, does not require a hospital stay, it provides advantages such as low cost and safe application in patients with gynecological or breast cancer since there is no hormonal treatment. I-PRF application is thought to be a promising and effective treatment option in patients with atrophic vaginitis complaints. However, many randomized controlled studies are needed to determine its long-term efficacy.
