Abstract (EN)

Aim; PT-Cy +/- ATG based GvHD prophylaxis makes it possible to perform HLA mismatched allogeneic transplantation. Selection of HLA mismatched locus, number of HLA mismatched locuses, presence of DSA, age of the donor, patient-donor sex mismatch, patient- donor relationship, ABO mismatch, CMV serostatus are taken into consideration for choosing the best donor. In this retrospective study we analyzed the role of immun and nonimmune parameters on transplant outcomes in haploidentical transplantation. Patients and Methods; Sixty-two patients with acute leukemia and MDS who underwent haploidentical transplantation were analysed retrospectively. All patients receieved peripheral blood stem cell transplantation with PTCy based GvHD prophylaxis and FluBu3 based conditioning regimen. Results; Number of HLA mismatched locus, -DQB1, -B-leader match, -DRB1, - DP match, patient-donor relationship, donor age ≥45 years, did not show any significant effect on transplant outcomes and survival. DP matched patients showed better OS. Two year OS was 49.4% and PFS 69.3%. Conslusion;Not the number of HLA mismatched areas but the locus of HLA mismatched area showed a significant effect on transplant outcome. PT-Cy based GvHD prophylaxis has an inreased risk of infectious comlications early after allogeneic transplantation.
