Frequency of cardiovasculer event development and related factors in preemptive renal transplant patients

Abstract (EN)
Cardiovascular diseases are the most common cause of death in both sexes worldwide. The prevalence of chronic kidney failure is an important public health problem that increases every year and the most common cause of death in this group of patients is cardiovascular diseases, with a risk approximately 10 times higher than the general population. Although this risk is reduced with renal transplantation, it has been determined that it is 3-5 times more than the society. If the risk of developing cardiovascular events can be predicted in patients whose renal reserve and quality of life improves with renal transplantation, it is thought that morbidity and mortality can be reduced by taking precautions and early intervention. It is aimed to determine the frequency and factors affecting the development of cardiovascular events in pre-emptive patients who do not have a history of cardiovascular disease since it is known that there is a high risk of entering dialysis and known cardiovascular disease in the development of cardiovascular event after renal transplantation. This study included 336 patients who preemptive renal transplantation, between 01 January 2007 and 31 December 2017 at Akdeniz University Medical Faculty Hospital Organ Transplant Clinic. Cardiovascular events were observed in 5.1% of the patients. Cardiovascular event development was statistically significant in patients with body mass index > 25 kg/m2, hemoglobin > 10 mg/dl and GFR > 10 ml/min/1.73 m² before transplantation (p <0.05). When evaluated by Cox regression analysis, only BMI and GFR were found to be independent risk factors. There was no statistically significant relationship between calcium, phosphate, parathormone, crp, urine protein / creatinine ratio, determination finding in favor of left ventricular hypertrophy by echocardiography (p> 0.05). The fact that the patients who were included in the study were preemptive and that they did not have any previous cardiovascular events cause the prevalence to appear less compared to the literature. With this study, it is revealed that importance of patients should reach their ideal weight before transplantation and should be attention paid to the group with GFH > 10 ml/min/1.73 m² as much as a group with GFH <10 ml/min/1.73 m².
