Evaluation of the effects of preemptive lornoxicam on the pain, swelling and body temperature after the impacted mandibuler 3. molar surgery

Abstract (EN)
The third molar surgery is the most common procedure in oral surgery, and pain and edema are the most encountred complications after the procedure. The advancement of the body temperature might be detected following operation. There are many treatment modalities after the third molar surgery to redound the life quality in the postoperative period. The aim of the present study was to evaluate the effects of preemptive lornoxicam on pain, edema and body temperature after the third molar surgery.A total number of 43 patients (14 men, 29 female) aged between 18-33 years requiring bilateral and symmetrically impacted lower third molar surgical operation were included in this randomized and double-blind study. The facial measurements and the body temperatures of the all patients were measured and recorded preoperatively. Firstly 8 mg I.V. lornoxicam was given in the preemptive group . The anesthesia of the nervus alveolaris inferior is provided after 25 minutes and the surgical removal of the impacted mandibular third molar was handled. The operation time, the amount of the anesthesic solution and the complications because of the operation or analgesic were recorded. The placebo was injected after the operation and the patients were re-called for the follow-up at postoperative 1. and 7. days. The same patient was appointed for the other impacted third molar one month later and the opposite protocol of the first operation was applied.Pain was evaluated by VAS and edema was evaluated by facial measurements using tape measuring technique. Pain assessment was pointed out the severity of pain in postoperative 12 hours, the first analgesic requirement, the total number of the analgesic in postoperative 5 days and the satisfaction of the patients. Facial measurements were handled by the total distance of tragus-mouth corner, tragus-menton and eye corner-gonion in preoperative and postoperative 1. and 7. days. Body temperature was measured using axiller technique in preoperative and postoperative 1. and 7 days.As the results of statistical analysis of the data, no statistical differences were observed on edema and body temperature between two groups (p?0.05). However, significant statistical differences were observed on pain particularly in the preemptive group (p<0.05). It is determined that preemptive group feels less pain especially in 5 hours period after the operation and needs fewer analgesics in postoperative 12 hours . It is also detected that the first analgesic required time after the operation is longer in the preemptive group and it is observed that there is a statistical difference between two groups. Although it is confirmed that the satisfaction of the patients in the preemptive group is higher than the postoperative group. There is also no complication related to lornoxicam.Preemptive analgesia is encouraged according to the results of this study. Increasing the analgesic effects of lornoxicam was observed in the preemptive group. According the results, preemptive lornoxicam can be an alternative, safe and preferable agent for the treatment of postoperative complications following third molar surgery.Keywords: Preemptive analgesia, impacted mandibuler third molar surgery, lornoxicam.
