Investıgatıon of adıponectın, leptın and apelın levels ın patıents wıth psorıasıs vulgarıs

Psoriasis is a chronic, inflammatory disease of skin, which proceed with T lymphocyte activation. It was reported that an increase was observed in the frequency of metabolic syndrome in individuals with chronic inflammatory disease, as chronic inflammation leads to atherogenesis and peripheral insulin resistance. Several important molecules such as leptin, adiponectin and apelin, which was shown to have a role in energy metabolism, released from lipid tissue and it was suggested that levels of these molecules altered in metabolic syndrome by increasing obesity. In this study, it was aimed to investigate potential relationship between these molecules and psoriasis.Fasting plasma glucose, triglyceride, HDL cholesterol and serum adiponectin, leptin and apelin levels were measured in 50 adult patient with psoriasis, 20 patients with pemphigus vulgaris and 46 healthy individuals. In addition, BMI, waist circumference and blood pressure were measured in patient and control groups.Metabolic syndrome frequency was found as significantly higher in patients with psoriasis than control group (p<0.05). It was found that serum adiponectin levels were significantly lower in patients with psoriasis than those in control group, while leptin and apelin levels were significantly higher (p=0.004, p=0.018 and p=0.003, respectively). Serum leptin levels were found as significantly higher in patients with pemphigus vulgaris than those in healthy controls (p=0.019). No significant difference was found between psoriasis patients with metabolic syndrome and without metabolic syndrome serum adiponectin, leptin and apelin levels (p>0.05, p>0.05 and p>0.05, respectively). Among the patients with psoriasis, PASI score was mild in 21 patients (42%), moderate in 23 patients (46%) and high in 6 patients (12%). It was found that DLQI index was significantly high in patients with high PASI score (p=0.002). A significant increase was observed in serum leptin levels in patients at risk regarding waist circumference (p<0.001). No significant difference was found between percentage involvement of body surface, PASI score, smoking and adiponectin, leptin and apelin values.In conclusion, our study demonstrates the frequency of metabolic syndrome in patients with psoriasis. We think that adiponectin, leptin and apelin may play role in the pathogenesis of psoriasis, but these adipokines aren?t markers, which are related to severity of disease.
