Radiological and laboratory factors related to 30-day mortality in pulmonary thromboembolism

Abstract (EN)
INTRODUCTION AND PURPOSE: Pulmonary thromboembolism (PTE) is a disease that requires rapid detection of mortality risk and is among the emergencies of the respiratory system. In our study, it was aimed to examine the relationship between demographic, clinical features, laboratory and radiological factors and 30-day mortality in patients with PTE confirmed by computed tomography pulmonary angiography, who were hospitalized in Chest Diseases, and to present new results that may help guide the management and treatment of PTE. METHOD: Sociodemographic, laboratory and radiological characteristics, electronic files, and archive files of patients hospitalized in the Chest Diseases Service and ICU with the diagnosis of pulmonary embolism between 01.01.2020 and 15.10.2022 were retrospectively analyzed. RESULTS: Of the 204 patients in our study, 75 (36.8%) were male. The median age of the mortal patients was 82 years old. The mortality was seen in 29 people (14.2%). The high risk of mortality according to the age of the patients, sPESI scores and ESC 2019 guidelines; presence of thrombolytic use in treatment; heart rate; low SaO2 and systolic blood pressure; GFR ≤ 60 mL/min/1.73 m2; high D-dimer, BUN, RDW and CRP levels, low GFR, albumin levels; When the high BUN-albumin, CRP-albumin and neutrophil-lymphocyte ratios were associated with 30-day mortality (p < 0.05). A similar, moderate and statistically significant correlation was found between D-dimer levels and thrombus load (p < 0.001). CONCLUSION: In pulmonary thromboembolism, the early mortality of the patients can be predicted with higher certainty by evaluating together the age, the aforementioned clinical and laboratory features.
