İntroduction: One of the most important factors affecting mortality and morbidity in colorectal surgery is anastomosis safety. In colorectal surgery, central vascular ligation is performed before the colon to be resected is transected and vascular flow of the colon is controlled after transection. With this approach, anastomosis safety is endangered in case of insufficient vascular flow of the segments to be anastomized after ligation and transection. Goal: In our study, we investigated the effect of central vascular ligation level on anastomosis safety in marginal artery occlusion in left colon surgery on shelves. Material and Method: In the study, it is planned to use 42 male Wistar albino rats, 250-280 gr. Rats will be divided into 3 groups: 1st group (n=14): Control group in which left colon transection and anastomosis was performed without vascular ligation 2.Group (n=14): The group that underwent left colon transection and anastomosis after inferior mesenteric artery high ligation and marginal artery ligation 3rd group (n=14): The group that underwent left colon transection and anastomosis after inferior mesenteric artery low ligation and marginal artery ligation The experiment will last for 5 days and at the end of the 5th day, all rats will be sacrificed. The presence of changes in the anastomotic line in the control group and in the anastomotic line in the other groups in sacrified animals will be demonstrated histopathologically by vascularization, epithelialization, inflammation evaluation and mechanically by burst pressure. Results: The results of the experimental animals were compared.In the burst pressure values, it was observed that the mean pressures of the low ligated group were higher than the high ligated group, but there was no statistical difference. When the reepithelialization values were examined, it was seen that it was higher in the low ligation group compared to the high ligation group, but it was also not statistically different. The mean values of the group with high ligation between inflammation score and microvascularity values were higher than the group with low ligation, but there was no statistically significant difference. Conclusion: As a result of our animal model study; we concluded that the high ligation of the CME and TME techniques we applied in left colon and rectal surgery can be safely applied in patients with marginal artery thrombosis without increasing the rate of anastomotic leakage and without preoperative or peroperative imaging, even if the marginal arch is occluded.
