Protective effects of hydrogen rich saline solution in ischemia reperfusion model due to experimental intestinal volvulus in rats

Abstract (EN)
SUMMARY Intestinal ischemia/reperfusion (I/R) injury occurs in clinical conditions such as NEC, strangulated hernia, inflammatory bowel diseases, mesenteric artery or vein occlusions, trauma, various types of shock, severe burns, intussusception and volvulus. In our study, hydrogen-rich saline solution (HZSS), which has antioxidant, antiapopitotic, anti-inflammatory and anti-allergic properties, was used on intestinal I/R damage due to volvulus and its protective effect was investigated. In the study, 30 healthy female Wistar Albino rats weighing between 220-250 g were divided into 5 groups. Median laparotomy was performed to all rats under anesthesia. Before the experiment, Sham group (Group 1) was given intraperitoneal physiological saline, HZSS group (Group 2) was given 5 ml/kg HZSS intraperitoneally and no action was taken. In the volvulus-I/R group (Group 3), a 5 cm small intestine segment from 2 cm proximal to the ileocecal valve was fixed by rotating 360º clockwise. After waiting for 2 hours of ischemia, a relaparotomy was performed, and the fixation suture was opened and a reperfusion period of 2 hours was expected. In the Treatment I group (Group 4), HZSS was injected intraperitoneally at a dose of 5 ml/kg just before reperfusion, and the fixation suture was opened for reperfusion and a reperfusion period of 2 hours was waited. In the Treatment II group (Group 5), HZSS was injected at 5 ml/kg intraperitoneally 1 hour before ischemia (volvulus) and I/R was performed. Blood samples and small intestine tissue were taken from all rats at the 2nd hour of reperfusion. TAS, TOS levels in blood sample and tissue and histomorphology in tissue were examined. In the Treatment I and Treatment II groups, TOS and OSI levels in the tissue were found to be statistically significantly lower than in the I/R group (p< 0.05). Although TAS and TOS levels in the blood sample in the Treatment I and Treatment II groups were not statistically significant compared to the I/R group, they were lower than the I/R group (p>0,05). As histomorphological, deterioration of villus structure, separation of epithelium and fragmentation of lamina propria were found to be statistically significantly lower in the Treatment I and Treatment II groups compared to the I/R group (p value 0,001). In this study, it has been shown through biochemical and histomorphological parameters that HZSS prevents intestinal damage in I/R damage due to volvulus.
