The effect of prolonged pneumoperitoneum on renal functions in rats

Abstract (EN)
The aim of this study is analysis of renal function loss, which occurs secondary to the increase of PNP time, under different pressure levels. 60 rats were separated into 10 groups. Groups are designed as group 1 Shamm group; groups 2, 3 and 4 are consecutively under 4,8 and 12 mmHg pressures with 60 minutes PNP; groups 5, 6 and 7 are consecutively under 4, 8 and 12 mmHg pressures with 120 minutes PNP; groups, 8, 9 and 10 are consecutively under 4, 8 and 12 mmHg pressures with 240 minutes PNP groups. In all groups it was observed that BUN values have statistically meaningful difference. It was identified that in 120 minutes PNP group with 8 mmHg pressure Cr values have statistically meaningful difference. It was observed that for IL-18 values there is no statistically meaningful difference in all groups. NGAL has statically meaningful difference in both high pressure groups and the elongated PNP groups. It was identified that pressure and time increase is two independent factors for the NGAL levels. For Cys-C the statistically meaningful difference is observed only in the highest pressure and the longest PNP group. Conclusions; serum IL-18 is not an effective biomarker in AKI. NGAL and Cys-C is a new and sensitive markers of early diagnosis of AKI. Although BUN and Cr were used for many years and sensitivities should not be ignored. We believe minimalization of laparoscopy time, and in elongated PNP situations keeping low pressure might contribute to preservation of renal functions.
