Effects of calcitriol on bone dansitometry and biochemical parameters inpatients with renal transplant on postrenal transplantation period

Abstract (EN)

Background and aims: Osteopenia and osteoporosis are commonly developing complications after solid organ transplantations. The characteristics of bone diseases developing after renal tranplantation are different than seen in other organ transplants. Duration of dialysis, severity of secondary hyperparathyroidism, dosage and usage period of immunsupresive drugs, renal functions and serum levels of vitamin D3 are main factors in determining the severity of bone disease. In this study we aim to investigate the bone mineral density and alterations by time in patients who had renal transplant. In addittion, we also investigate the effects of vitamin D3 on bone disease and biochemical markers.Methods: For this purpose, we assessed the renal functions, serum Calcium, Phosphorus, Parathormone, Magnesium, Osteocalsin, C-telopeptidis levels and dansitometric measurement of femur and vertebra of all patients at the begining and at the end of study. This parameters are considered seperately in patients who were taking vitamin D3. We conducted 57 male, 28 female totaly 85 patients into this study. All patients have been under medical observation in Çukurova University Faculty of Medicine department Nephrology. The mean age of the patients were 36.25±10.5 years, mean period after tranplantation were 38.2±30.2 months and the mean follow up period were 9.82±2.72 months. 57 of 85 patients were given to take Vitamin D3 according to their serum calcium level.Results: During follow up period we found statistically significant improvement in T score of vertebra and femur(p<0.05). We also found statistically significant improvement in T score of femur and vertebra T score of patients who were taking Vitamin D3 (p values are 0.038, 0.021 respectively). During study period we found statistically significant decrease in serum Osteocalsin levels in both all patients and patients who were taking Vitamin D3 (p< 0.05). There were no difference in serum Calcium, Phosphorus, Parathormone, Magnesium, C-telopeptidis levels. İnterestingly we observed statistically significant decrease in serum total cholesterol, High density lipoprotein and tryglycerid levels in patients who were taking Vitamin D3 (p<0.05).Conclusion:. We find improvement in bone mineral density by time in renal tranplant recipients. Taking Vitamin D3 has positive effects on vertebra T score and serum tryglycerid levels. Because of the positive effects of Vitamin D3 on both bone and lipid metabolism, despite of improvement in bone mineral density without treatment in following period after renal transplantation, vitamin D3 taking can be appropriate attempt for the patients who do not have hypercalcemia.Keywords: Renal Transplantation, Hyperlipidemia, osteocalsin, osteoporosis, vitamin D3
