The psychological situation and changes of the women giving birth in caesarean operation and vajinal birth and their comparison

Abstract (EN)

Giving birth appears in itself a stress factor for every woman. In this study, it is aimed to evaluate the probable different mental status change between the women who give birth by cesarean section and normal vaginal birth and to compare them. In addition to being very popular of this topic, it is very essential to be focused on the topic due to not having determined certain risk factors. If mental status changes exist, to investigate will add authenticity to this issue.In this study, 200 voluntary mothers are included who gave birth normally or by caesarean section birth in Izmir 9 September University Faculty of Medicine Department of Pediatrics Well Baby Clinic between the dates 25.01.2010 ? 30.06.2010, who have 0-3 month old healthily baby and who are between the ages of 15-49 (child bearing age). In this research, as a source of data, a questionnaire was created for Socio-demographic Characteristics, Personal Characteristics and Properties of Mental Status of mother. The data were obtained by face to face interview with participants who are selected for sample and by taking a written response to questionnaire. The obtained data, as a result of research, are evaluated by using appropriate statistical methods in SPSS 15.0 package programme. In addition, the Chi-square test and cross-tables were used for statistical comparison of descriptive data.The study involved 200 participants, 110 of them (55.5%), cesarean section, 90 of them (45.5%) gave birth to babies of normal birth.Psychiatric illness and general diagnostic systems (DSM-IV and ICD-10) of psychiatric disorders in pregnancy and the postpartum period were not defined as a distinct clinical diagnosis. Mood disorders occurred in postpartum period are collected in three major groups by classifying them according to the severity of symptoms, characteristics, treatment and prognosis as ?maternity blues (maternity blues, baby blues, postpartum blues)?, ?postpartum depression (post-natal depression)? ve ?postpartum psychosis?.In the study 65.5% (200/131) of the participants stated that they have maternity blues symptoms. 12% of them stated they have desolation during the postpartum period, 7.5% of them stated having experienced intense sadness, 13,5% of them stated having experienced insomnia, 24% of them stated having experienced anxiety-tension, and 8.5% of them stated having other problems (inability to concentrate , do not cry ... etc.). 34.5% of the participants stated that they did not face problems after birth. As a result of the statistical evaluation, no significant maternity blues symptoms difference was observed between the attendees who gave birth normally and who gave birth by the caesarean (p>0,05).In the context of this study, it is determined that 72% (200/144) of the participants have symptoms of postpartum depression, 7% of them have eating disorder, 9% of them have sleep disorder, 7% of them have nervous breakdown, 4% of them have loss of energy and motivation, 2.5% of them have problems of Inability in concentration, 12.5% of them feel hopeless and despair, 11% of them have social withdrawal and crying, 6.5 % of them have loneliness and fear, 2% of them have the sense of guilt and worthlessness, 8.5% of them have sexual aversion, and 2% of them have other distresses. 28% of the participants reported no symptoms of depression. As a result of the statistical evaluation, no significant postpartum depression symptom was observed between the attendees who gave birth normally and who gave birth by the caesarean birth. (p>0,05).The participants put into words to the question asked in order to assess the postpartum psychosis of the participants who are in the scope of our study that they do not have any sense of such as disinterestedness with their babies, apathy to their babies, feelings of inadequacy or any thoughts such as self harming of harming to their babies, only 9% of the participants stated that they have problems such as non acceptance of their babies. The reason in the 9% of the participants who have the feeling of non acceptance of their babies are resulting from having unplanned baby, giving premature birth and being inexperienced in baby care.It is seen that a majority of the participants are affected morally in adaption to new one with old life style because of postpartum period means for the mother transition to a new life, parenting preparation period. The participants who transited to post birth period were told about every women might have such as complaints. In case of having more complaints, the necessity of going to psychiatric clinic of the nearest hospital is told to the participants.Key Words: 1. Caesarean Operation 2.Vaginal Birth 3.
