Right ventricular failure experience after LVAD implantation

Abstract (EN)
Objective: We aimed to present mid – term results of total correction in our TOF patients. Material and Method: We carried out totally 104 total corrections in TOF patients between January 2006 and August 2011. While mean age of cases was 4,1 (1 – 24) male:female ratio was 60:44. Right ventricular outflow tract reconstruction was performed by transannular patching, resection from right atriotomy, and contegra graft respectively in 89, 12, 3 patients. V-Plasty technique was also performed in 10 patients of 89 transannular patch patients. Mean follow-up time was 26,4 months (6 – 62). Patients were followed-up by echocardiography to determine pulmonary insufficiency, transpulmonary gradient, ventricle functions, and residual VSD. Results: However we had 7 early term mortality, there were no late term mortality amongst all patients. Permanent pacemaker was implanted to 2 patients with complete and 1 patient with Mobitz Type-2 atrioventricular block. Residual VSD was not observed. Nine patients who had severe pulmonary insufficiency underwent to off-pump injectable pulmonic valve replacement. Conclusion: Although early tem results of total correction in TOF surgery are satisfactory; pulmonic valve insufficiency, right ventricular dilatation, and ventricular arrhythmias have been reported as late term complications. In this context, reconstruction of right ventricular outflow tract according to appropriate size of body surface area and preservation of native valve in possible cases are important determinants to prevent early term morbidity-mortality and late term complications. Nonetheless, off-pump injectable pulmonic valve replacement that was performed to 9 patients in our center should be considered when postoperative pulmonary insufficiency was observed.
