Abstract (EN)

THE RELATİONSHİP BETWEEN THE İNCİDENCE OF ATTENTİON DEFİCİT HYPERACTİVİTY DİSORDER FREQUENCY AND SUBTYPES İN THE GROUP WİTH AND WİTHOUT COMORBİD PERSONALİTY DİSORDER İN THE PATİENTS WİTH SOCİAL ANXİETY DİSORDER Objective and hypothesis Purpose of the study; The comparison of SAD and SAB + AVDP groups with sociodemographic and clinical features, as well as ADHD frequency and subtypes. The hypothesis of the study is that there is a relationship between the incidence of attention deficit hyperactivity disorder frequency and subtypes in the group with and without the comorbid personality disorder comorbidity in social anxiety disorder patients. Method: This research is an analytical cross sectional study. The collection of research data was carried out between February 2017 and July 2018 at the Aydın Adnan Menderes University Research and Practice Hospital Psychiatry Policlinic and Psychiatry Inpatient Department. The universe of the study was all SAD and SAD+ADHD patients who were admitted to the polyclinic or were being followed up during the period of finding participants for the study. A total of 28 patients, including 13 SAD patients and 15 SAD+ADHD patients, were included in the study. The Sociodemographic Clinical Data Form, Structured Clinical Interview for DSM-IV Axis I Disorders(SCID-I), Wender-Utah Rating Scale, Beck Depression Inventory, Beck Anxiety Inventory, Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version Turkish version, Liebowitz Social Anxiety Scale, and DSM-IV based Adult ADD/ADHD Diagnostic and Evaluation Inventory were applied to all participants. The Type-1 error level was taken as 5%. Results: In socio-demographic comparison of SAD and SAD + AVPD groups; there was no significant difference except the history of substance use. History of substance abuse was higher in the SAD group. In the SAD + AVPD group, ADHD was found higher than in the SAD group, although the difference was not statistically significant (p: 0.052). There was no statistically significant difference between the SAD + AVPD co-diagnosed group and SAD in terms of ADHD subtypes and frequency (p: 0.109).In the SAD + AVPD group, there was a higher rate of childhood ADHD. This difference was statistically significant (p: 0.042). In the SAD + AVPD group, there was a higher incidence of childhood SAD.This difference was statistically significant (p: 0.001).When we compared the SAD group and the SAD + AVPD group in the terms of the SAD subtypes , the SAD+AVPD group had statistically significantly higher frequency of Generalized Social Anxiety Disorder Subtype(p: 0000).In the SAD + AVPD group, higher comorbid psychiatric disorder was found (p: 0000). 44.3% of the sample was diagnosed as having ADHD. We found that 80.6% of patients with ADHD had Predominantly Inattentive subtype and 19.3% of them had a combined subtype. There was no patients diagnosed as Predominantly Hyperactive or Hyperactive-Impulsive subtype in our sample. We foundthat 72.9% of the patients had generalized SAD subtype and 27.1% had non-generalizedsubtype in our sample. Conclusion: In our study, we found that there was no statistically significant difference in terms of ADHD frequency and subtypes between the two groups with SAD and SAD + AVPD, although ADHD diagnoses were higher in the SAD + AVPD group. Childhood SAD and ADHD diagnoses were found to be statistically higher in the SAD + AVPD group. For this reason, treatment of childhood ADHD and SAD are predicted to prevent the development of AVPD.There is a need for new studies about childhood SAD and ADHD on a larger scale.
