The effect of admission ECG findings on prognosis in patients with adiagnosis of ST segment elevation myocardial infarcts

Abstract (EN)

In a patient who is admitted to the emergency room with chest pain, ACS is a group of diseases that should be diagnosed or ruled out. Emergency physicians should be able to quickly and appropriately perform interventions, especially focused history taking, adequate physical examination, ECG interpretation and risk assessment of the patient, for the diagnosis of ACS. In our study, 399 patients with STEMI, who applied to Akdeniz University Hospital Emergency Service between 01.01.2018 and 31.11.2019, were included. Among these patients, after excluding the patients who did not have an ECG record and did not have angiography at the end, the demographic data of 333 patients were examined; their ECG findings and the coronary vessels treated in angiography were noted, and the patients' in-hospital deaths, first month deaths, recurrent myocardial infarctions and discharge from the hospital outcomes were noted. It was found that 270 of our patients were male (80,8%) and 63 were female (18,9%). The mean age of our patient group was found to be 61,87 (54-72). During the patient follow-up, it was determined that 33 patients died in hospital and in the first month. Early death was found to be correlated with patient's age, systolic blood pressure at the time of admission, heart rate, amount of reciprocal ST segment depression in millimeters, and length of hospital stay. For this reason, vital values of the patients at the time of the hospital admission should be carefully examined and ECG should be taken as soon as possible. To prevent complications that may occur, early reperfusion and appropriate treatment may increase survival rates.
