Abstract (EN)
INTRODUCTION AND AIM: In our study, it is aimed to reveal whether there is a difference in the pathophysiological mechanisms that initiate the destabilization of coronary plaque between these groups by grouping the patients as patients presenting during the day and night according to the circadian distribution, in our study. MPO, hsCRP and NE activities have been shown to destabilize plaque and predispose to plaque rupture. Therefore, the aforementioned surrogate biomarkers were used in our study. MATERIALS AND METHODS: 88 patients who came to our hospital with the diagnosis of STEMI and underwent primary PCI were included in the study. When blood flow was provided in the totally occluded artery causing myocardial infarction, arterial blood was drawn from the procedure site. Samples were taken for measurement of MPO, MMP-1, NE, hs-CRP levels. RESULTS:. At the end of the evaluations, among the patients grouped as day and night according to the circadian distribution, MMP-1 (6.8 [ 1.8-26.4 ] vs, (7.5 [1.8 - 30.8] p=0.203), MPO (4.1 [1.8 - 12.1] vs, (4.4 [1 - 10.8] p=0.280), hs-CRP (5.7 [2.5 - 15.5] etc, ( 5.4 [0.8 - 15.2] p=0.370) NE (14.1 [2.4 - 31.3] vs, (13.4 [5.3 - 65.3] p =0.854) No significant difference was found between. CONCLUSION: In our study, no significant difference was found among the available biomarkers. A larger number of patients and to get more detailed information about plaque morphology may help us to reach more meaningful and precise results. For this reason, there is a need for clinical studies using the above-mentioned methods and following larger case groups. Keywords: Circadian distribution, inflammation, plaque rupture
