The relationship between peripheral blood monocyte chemoattractan protein-1 levels and instent restenosis in patients with stabile angina pectoris undergoing stent implantation in one coronary vessel

8) SUMMARY:Purpose: Monocyte Chemoattractant Protein-1 plays an importantrole in atherosclerotic process by attracting monocytes to damaged vascularsite. It may also act with the same mechanism in restenosis developmentafter stent implantation. The purpose of the current study is to test thehypothesis that increased plasma MCP-1 , serum hsCRP and peripheralblood monocyte counts are correlated with an increased restenosis risk.Materials and Methods : 34 patients (25 male, 9 female) with StabileAngina Pectoris were included in the current study. Patients were evaluatedwith control quantitative coronary angiography for the presence ofrestenosis at sixth month after stent implantation.> 50% stenosis indiameter is accepted as restenosis. Correlations between instent restenosisand MCP-1, hsCRP and monocyte levels at basal, first month and sixthmonths post stent implantation were investigated.Results: Instent restenosis was present in 8 (23,5%) patients at the endof sixth months. Minimum luminal diameter in restenosis group at sixthmonths was 0,97 +/- 0,22 mm whereas 2,03 +/- 0,52 mm in no restenosisgroup. Minimum luminal diameter in restenosis group post stentimplantation was 2,37 +/-0,22 mm whereas 2,49 +/- 0,41 mm in norestenosis group. There was no statistically significant difference betweentwo groups in basal, first month and sixth months plazma MCP-1, hsCRPand monocyte levels.52Conclusions: Although the relationship between instent restenosis andMCP-1 was shown previously, this relationship is not confirmed in ourstudy. We did not detect any relationship between hsCRP levels and instentrestenosis which had conflicting results in previously published literature.We did not detect any correlation between blood monocyte count andinstent restenosis which was shown to be present in a single previous study.It is concluded that MCP-1, hsCRP and peripheral blood monocyte countdo not provide any clinical benefit in predicting restenosis.Key Words: Instent restenosis, Monocyte Chemoattractant Protein-1,hsCRP, monocytes, percutaneous coronary intervention.53
