Research of the relevance between umbilical coiling index, fetal cerebral and umblical artery doppler parameters, umblical vein blood gas and IMA values in term pregnants

Abstract (EN)

Purpose: The aim of our study is to determine whether there is a significant relationship between the Ischemia Modified Albumin value, which can also be considered as an indicator of oxidative stress in newborns, the umbilical vein blood gas value, which is one of the first indicators of fetal well-being, and the umbilical coiling index, umbilical and middle cerebral artery Doppler parameters, which are values that can be measured antenatally. Material and method: Within the scope of the research, 65 pregnant women who applied to the Department of Obstetrics and Gynecology for pregnancy follow-up and were hospitalized and delivered, who had a pregnancy of 37 weeks or more (term) and who did not have an additional diagnosis that could be considered a risk factor, were examined with prenatal ultrasound and umbilical coiling index, umbilical and middle cerebral artery Doppler parameters were recorded. Blood gas and serum samples taken from the umbilical vein during birth were sent to the biochemistry laboratory and blood gas values and IMA (Ischemia Modified Albumin) were studied and recorded. Age, gravida and parity number of pregnant women, week of birth, birth weight of newborns, 5th minute APGAR score, whether they were followed in the neonatal intensive care unit and gender were recorded. Obtained data were evaluated using IBM SPSS 11.5 statistical program. Results: In umbilical cord coiling index measurements, the lowest measured value was 0.210 coil/cm, the highest measured value was 0.471 coil/cm, and the average coiling index value was 0.337. Umbilical and middle cerebral artery Doppler parameters were not observed differently from the reference values. No significant correlation was detected between the coiling index and umbilical vein blood gas values, IMA values, or umbilical and middle cerebral artery Doppler indices and gestational age, gestational week, or 5th minute Apgar score. There was a negative correlation between umbilical artery S/D ratio and newborn birth weight (p<0.05), negative correlation between umbilical artery RI value and newborn's birth weight (p<0.05), there was a positive correlation between the umbilical vein pH value and newborn's 5th minute APGAR score (p<0.05) and a negative correlation (p<0.05) between the umbilical vein pCO2 value and the newborn's 5th minute AGAR score. It was found that those with umbilical vein blood gas values below pH 7.32 had a higher rate of having had a cesarean delivery, and those with pH lower than 7.32 had a higher rate of being followed up beside their mothers. While umbilical artery S/D and PI were on average higher in newborns in neonatal intensive care, pH was found to be lower. Conclusion: In order to evaluate the relationship between coiling index, umbilical vein blood gas, IMA and umbilical and middle cerebral artery Doppler parameters which are antenatal markers of fetal well-being, more clearly; further studies are needed with populations with different pregnancy complications.
