Abstract (EN)

Relation of Histologic Types to Prognostic Parameters in Testicular Tumors Testicular tumors are rare and constitute 1-2% of malignant tumors in males. It is seen in mainly young men and is peaking in the fourth decade. Germ cell tumors are the most common and the most important tumors of this region. The distribution and frequency of subtypes varies by age. Generally, they occur as unilateral painless masses in the testis. In this study 92 cases who underwent orchiectomy for testicular neoplasm at our institution between 2005 and 2016 were included. Age, tumor size, tumor histology, histologic type, presence of intratubular germ cell neoplasia, tumor extension, pathological tumor stage, and surgical margin were evaluated. Clinical findings of the cases were obtained from pathology archive reports. Tumors were reclassified according to the 2016 World Health Organization Classification of the Testicular Tumors. Upto our knowledge this was the first epidemiological study in this region. Analysis of each regions statistical information reflecting its own profile is also important. The relationship between the prognostic parameters and types of testicular neoplasia may give an idea about the prognosis of the disease in its region. In this study, it is also aimed to contribute to the determination of the distribution of testicular neoplasms according to new classification in our region. The frequency distribution of tumor groups was consistent with the literature and there were differences in percentage distribution. Seminom (41%) was the most common germ cell tumor and mixed germ cell tumor was the second most frequent (35%). In the literature, pure germ cell tumors were found to be second after seminom but tumors were found to be embryonal carcinomas. However, second germ cell tumors observed in our series were found as Prepubertal type Teratoma. Seminoma and Mixed Germ Cell Tumor diagnosed cases were evaluated in terms of whether there were any significant differences by comparing all parameters. Significant differences were found in patient age, perineum fat tissue invasion, lymphovascular invasion and necrosis parameters. The mean age of seminomic lesions was 35.5 and the number of patients with mixed germ cell tumors was 28, and it was determined that seminomic lesions were older than mixed germ cell tumors (p: 0.024). Peripheral fat tissue invasion, lymphovascular invasion and necrosis parameters were found to be higher in the MGHT group than in the Seminoma group. It was seen that the general characteristics of the cases in our series were in great agreement with the literature findings.
