Tinnitus patients? treatment with music in sleeping

Abstract (EN)

According to the neurophysiological model of tinnutus, tinnutus signals by establishing a conditioned reflex connections between limbic and the sympathetic portion of autonomic nervous system may cause stress. Limbic and autonomic connections disorders such as anxiety, panic attacks and reduction in recreational activities cause to emerge tinnutus.The aim of this study based on neurophysiological model in patient with tinnitus is to create a acoustic desensitization against tinnitus signals by using the relaxing effect of music thus helping the development of habutuation in the central nervous system against tinnitus signals.This study included the people having tinnitus at least 1(one) month and the people suffering from tinnitus and didn't benefit from other treatment techniques in age between 23-65 years(mean 44,2+- 11,1 years), 14 women(%46,7) and 16 men (%53,3).Questionnarie forms were asked to fill (demographic, clinical information, TEA, BDA),odiologic tests were performed and tinnutus parameters (frequency, severity, minimal masking level, residual inhibition) were measured before, during and after the treatment. Preferred music by the patients integrated into the center frequency of their tinnitus were accepted as a center frequency, generated narrow band noise with the reference of ANSI 2004 narrow band noise lower/upper frequency limit,were provided to listen during their sleep at night during a period of 6 months.No significant difference was found in tinnutus parameters between male and female patients( p>0,05). There wasn't a significant change in their tinnutus frequency( p>0.05). There was a statistically significant improvement in the psychoacoustic parameters of tinnutus(severity, residual inhibition, minimal masking level) ( p<0,05 ) at the end of the treatment. There was a significant improvement in the score of TEA, BDA and VAS (frequency and duration of tinnutus, disturbance degree, attention deficit related to tinnitus, sleep problems related to tinnutus) ( p<0,05 ) at the end of the treatment. 2 of the patient( 6,7%) said their tinnutus finished, 4 of them (13,3%) said their tinnitus reduced significantly, 10 of them (33,3%) said their tinnitus reduced, 4 of them (13,3%) said their tinnitus didn't change but not bother them and 10 of them (33,3%) said their tinnitus continued in the same way at the end of the treatment.As a result, acoustic desensitization gave succesful results for the 67,7 % of patient with tinnitus and caused to decline of the reaction agaist tinnitus.
